With the arrival of summer, skin tinting with dew becomes relevant. For many, tanned skin is a symbol of health, beauty and relaxation. But is it really so?
What happens in the skin during tanning, what effect does the sun have on the endocrine system, is there a safe tan and what is currently proven by science?
What is tan?
A tan is actually the skin's protective reaction to the sun's ultraviolet (UV) radiation.
These types of rays penetrate the skin and cause damage to its cells at high intensity. In response, melanocytes are activated, which produce melanin in order to protect themselves.
Melanin can absorb and partially neutralize UV radiation, thus helping to protect skin cells from further damage. It is the increase in the amount of melanin that causes darkening of the skin, i.e. the formation of a tan.
Ultraviolet (UV) radiation is a spectrum of light coming from the sun. Its UVA and UVB rays can damage the DNA of skin cells.
DNA is each cell's "instructions" that determine how the cell should work, grow, and reproduce.
Under normal conditions, the body has DNA repair mechanisms that repair most of these lesions. However, if the damage is frequent or very intense, not all errors can be corrected. As a result, permanent changes remain in the DNA — mutations.
If such mutations accumulate in genes that control cell growth, division, and destruction of damaged cells, the cell can lose control of growth and eventually develop skin cancer, including melanoma.
Darkening of the skin means that the cells have already been exposed to UV radiation and are trying to protect themselves from further damage by producing melanin. However, melanin can neither provide complete protection nor restore DNA damage already received.
Ultraviolet radiation is a proven carcinogen. According to the classification of the World Health Organization's International Agency for Research on Cancer (IARC), solar ultraviolet radiation belongs to Group I carcinogens, which means that there is sufficient evidence of its cancer-causing activity in humans.
In addition, UV radiation is also associated with an increased risk of premature aging, photodamage, wrinkles, sagging skin, and dark spots.
Important facts
Cloudy weather does not mean safety. Clouds block only a small fraction of ultraviolet (UV) radiation, so about 80% of UV rays still reach the Earth's surface. Therefore, sun protection is important even in cloudy weather.
Damage from UV radiation is cumulative. Each period spent in the sun and each sunburn adds to the previous damage and there is a cumulation of DNA damage.
Childhood sunburns are particularly dangerous. Studies have shown that childhood and adolescent sunburns, especially blistering sunburns, significantly increase the risk of developing melanoma in adulthood.
Although the population is more or less informed about the high risks of these possible negative effects of tanning, it still does not lose its popularity.
Beta-endorphins may contribute to this - this is a substance that, in response to ultraviolet rays, leads to the emergence of positive feelings, self-confidence, and to some extent even the formation of an addiction to tanning.
Is there a positive side to tanning?
Although excess UV radiation is associated with significant risks to the skin, sunlight also has several important physiological effects on the body.
Vitamin D synthesis in the skin
Exposure to the sun's ultraviolet B (UVB) rays produces vitamin D in the skin. Vitamin D is important:
- for bone health
- for muscles
- for immune system
- for calcium and phosphorus.
It also plays a positive role in carbohydrate metabolism.
It is important to note that intense tanning or sunburn is not necessary to get vitamin D.
Sunlight and the sleep-wake cycle
Sunlight plays an important role in the regulation of the organism's biological clock - circadian rhythm.
Getting enough natural light during the day helps the brain to correctly determine the day-night cycle, which contributes to the natural production of melatonin in the evening and improving the quality of sleep.
Melatonin is a hormone involved in sleep-wake regulation.
Sun light and mood
Exposure to sunlight is associated with changes in the activity of neurotransmitters in the brain, including serotonin.
Serotonin is involved in the regulation of mood, emotional state and sense of well-being. That is why many people feel more energetic and positive on sunny days.
Sun lights and hormonal balance
The relationship between sunlight and various hormonal processes is a subject of active research. There is some evidence that seasonal changes and sun exposure may be associated with changes in certain hormone levels.
It is important that intensive tanning is not necessary to obtain these positive effects.
Does tanning affect the thyroid gland?
Hypothyroidism and hyperthyroidism
There is currently scientific evidence and confirmation that tanning in the sun affects the function of the thyroid gland - increases or decreases its production of hormones or changes the effectiveness of levothyroxine in the treatment of hypothyroidism.
if Hypothyroidism is properly controlled and the patient feels well, being in the sun is not limited, following general recommendations.
However, there are several factors to consider:
- Some people with hypothyroidism may have increased or decreased sensitivity to heat, depending on the individual.
- Dry skin, which is a characteristic symptom of hypothyroidism, may become more pronounced with intense sun exposure.
- If the patient has other diseases at the same time or is taking medications that cause photosensitivity, the sun exposure recommendations may differ.
In the case of hyperthyroidism the problem may be that at this time people often have heat intolerance, excessive sweating and increased body temperature. Therefore, discomfort may be more pronounced during hot summer days or prolonged exposure to the sun.
Patients whose disease is not yet well controlled should be especially careful, so that high temperature and dehydration do not aggravate the general condition.
During hyperthyroidism it is recommended:
- avoid long exposure to direct sunlight, especially during the hottest hours of the day;
- Drink enough fluids;
- Wearing light, light-colored clothing;
- Use of sunscreen.
If the hyperthyroidism is properly controlled and the patient feels well, being in the sun is not prohibited by following the general recommendations.
Autoimmune diseases
Although UV radiation affects the function of the immune system, currently there is not enough evidence to confirm that tanning in the sun improves or worsens the course of autoimmune endocrine diseases. Mechanisms by which UV radiation may affect autoimmune processes have been described in laboratory studies, but these results have not been translated into clinical practice. Hashimoto's thyroiditis, Graves' disease, Addison's disease and Type 1 diabetes has not yet been confirmed.
Graves' orbitopathy
With Graves' disease, some people develop Graves' Orbitopathy (eye disease), in which the tissues of the eye socket become inflamed. Patients may experience:
- Dry eyes;
- Burning or sandy sensation;
- Tearing;
- Increased sensitivity to light (photophobia);
- Incomplete closure of the eyelids;
- Protrusion of the eyes (exophthalmos).
Bright sunlight, wind, and a dry environment can make these symptoms worse.
Therefore, it is recommended for patients with Graves' orbitopathy:
- Using quality sunglasses that provide 100% UVA and UVB protection;
- Using artificial tears on the recommendation of a doctor if you suffer from dry eyes;
- Avoid prolonged exposure to the sun and strong winds;
- Drink enough fluids, especially in hot weather.
Thyroid nodules
Studies conducted to date have not shown that UV radiation from the sun promotes the growth of thyroid nodules, the formation of new nodules, or the transformation of a benign nodule into a malignant one.
Therefore, The presence of thyroid nodules in itself is not a reason to limit exposure to the sun. In this case, the recommendations are the same as for any person: avoid intense sun exposure, use sunscreen and avoid sunburn.
If the nodule is malignant or the patient has undergone thyroid surgery, limiting exposure to the sun is usually not necessary either. However, In the postoperative period it is important to protect the surgical scar from direct sunlight, because ultraviolet radiation can make the scar darker and more visible. For this purpose, it is recommended to use sunscreen with a high SPF or to cover the scar with clothing, especially during the first 6-12 months after surgery.
Diabetes mellitus and hot weather
High temperatures and prolonged sun exposure are of particular concern to people with diabetes, as heat can affect both blood glucose levels and the effectiveness of diabetes medications.
The risk of dehydration
In hot weather, the body loses a large amount of fluid through sweating. In people with diabetes, especially when blood glucose is high, the risk of dehydration is higher because excess glucose increases fluid loss through urine.
Dehydration, in turn, can cause blood glucose levels to rise and make diabetes more difficult to control.
How does heat affect insulin?
Insulin and some other diabetes medications are sensitive to high temperatures. Overheating insulin or leaving it in direct sunlight can change its action and reduce its effectiveness.
Insulin shouldn't be stored:
- Direct sunlight;
- In a closed, hot car;
- In a place where the temperature is high.
Heat can alter glucose levels
High temperatures, physical exertion in the heat and dehydration affect blood glucose levels. Therefore, during the summer it may be necessary to More frequent glucose control.
Special attention is required:
- During insulinotherapy;
- During prolonged physical activity;
- When traveling or in hot climates.
Practical tips for people with diabetes in hot weather
- Drink plenty of fluids unless your doctor has prescribed a fluid restriction.
- Monitor your blood glucose levels regularly.
- Store insulin and other medications as directed.
- Avoid peak solar activity (around 11:00 AM-4:00 PM).
- Wear light, breathable clothing and use sunscreen.
Modern diabetes management technologies, such as continuous glucose monitoring (CGM) systems and insulin pumps, greatly simplify glucose control. However, high temperatures and direct sunlight can affect their performance.
CGM sensores
High temperature, excessive sweating and sunlight may:
- Reduce the effectiveness of the adhesive layer of the sensor and cause its premature removal;
- Increase the risk of skin irritation;
- In some cases, it may affect the accuracy of the device.
It is important to protect the sensor in accordance with the manufacturer's recommendations during swimming, intense sweating or prolonged sun exposure.
Insulin pumps
The insulin in the insulin pump is sensitive to high temperatures. Overheating may reduce the effectiveness of insulin, causing blood glucose to rise.
Special care is required:
- When leaving the pump in direct sunlight;
- While on the beach or in a closed car;
- When staying in a very hot environment for a long time.
Practical recommendations
- Do not leave the CGM device, pump, or insulin in direct sunlight.
- Do not leave diabetes devices in a car in hot weather.
- If necessary, use special protective bags or thermal cases.
- If glucose readings suddenly change and the cause is unclear, consider the possibility of insulin overheating or a device problem.
It is important to remember: the heat is not a reason for a person with diabetes to refuse to travel, be in the sun or do activities, although extra caution and the correct use of technology are required in the summer.
Solarium
Tanning beds are a source of artificial ultraviolet (UV) radiation that became widespread in the late 1970s and 1980s.
Ultraviolet radiation from both the sun and tanning beds is classified as a Group I carcinogen by the World Health Organization's International Agency for Research on Cancer (IARC).
According to studies, starting to use a solarium before the age of 35 increases the risk of developing melanoma by approximately 47%. In addition, the risk increases with the frequency of tanning bed use and the total dose of UV radiation received.
Solarium is also associated with:
- with an increased risk of developing other malignant skin tumors;
- premature skin aging (photoaging);
- with faster development of wrinkles, pigment spots and loss of elasticity;
- risk of eye damage if appropriate protective equipment is not used.
Sunbeds are not a recommended source of vitamin D. If necessary, the vitamin D level should be corrected with the doctor's recommendation and with the help of drugs.
Auto tanning
Self-tanning is a cosmetic product that gives the skin a tanned look without exposure to sunlight. Unlike real namzeur, it does not stimulate the production of melanin and does not represent a reaction of the skin to ultraviolet radiation.
How self tanning products act
The main active ingredient in most self-tanning products is dihydroxyacetone (DHA).
DHA reacts with amino acids in the most superficial, already dead skin cells to produce brown pigments.
Discoloration usually begins 2-4 hours after application, reaches maximum intensity at about 24 hours, and gradually fades over 3-7 days in accordance with the natural exfoliation of the skin.
Is self-tanning safe?
According to current data, self-tanning containing DHA is considered safe if used correctly on the skin.
However, the US Food and Drug Administration (FDA) approves DHA for external use only.
This means that the product should not get into the eyes, lips, mouth, nose and other mucous membranes.
Special care should be taken when using in the form of a spray, as the aerosol can be inhaled. The long-term safety of DHA obtained in this way has not been fully studied, so it is recommended to avoid inhalation and contact with mucous surfaces.
The coloring obtained by self-tanning is only a cosmetic effect and does not provide protection against ultraviolet radiation.
If SPF is not indicated on the product label, it is necessary to use a separate sunscreen before going out in the sun.
- უნაყოფობას;
- ფარისებრი ჯირკვლის დაავადებებს;
- ტესტოსტერონის ან ესტროგენის კლინიკურად მნიშვნელოვან ცვლილებებს;
- მენსტრუალური ციკლის დარღვევას;
- თუთიის ოქსიდი (Zinc oxide);
(Titanium dioxide).
- დაუყოვნებლივ შეწყვიტოთ მზეზე ყოფნა;
- დაიდოთ გრილი (არა ყინულივით ცივი) კომპრესები;
- გამოიყენოთ მსუბუქი, უსუნო დამატენიანებელი კრემი ან ლოსიონი;
- მიიღოთ საკმარისი რაოდენობის სითხე;
- საჭიროების შემთხვევაში, თუ უკუჩვენება არ არსებობს, გამოიყენოთ არასტეროიდული ანთების საწინააღმდეგო პრეპარატი (მაგალითად, იბუპროფენი) ტკივილისა და ანთების შესამცირებლად;
- ბუშტუკები სხეულის დიდ ნაწილს მოიცავს;
- აღინიშნება მაღალი ტემპერატურა, შემცივნება ან ძლიერი სისუსტე;
- გაქვთ თავბრუსხვევა, გონების დაბინდვა ან ცნობიერების ცვლილება;
- აღინიშნება დეჰიდრატაციის ნიშნები (ძლიერი წყურვილი, პირის სიმშრალე, იშვიათი შარდვა);
- დაუყოვნებლივ გადაიყვანეთ ბავშვი ჩრდილში;
- გააგრილეთ კანი გრილი კომპრესებით ან გრილი შხაპით;
- უზრუნველყავით საკმარისი სითხის მიღება;
- დამწვრობა მიიღო 1 წლამდე ასაკის ბავშვმა;
- გაჩნდა ფართო ბუშტუკები;
- ბავშვს აქვს მაღალი ტემპერატურა, ძლიერი ტკივილი ან შემცივნება;
- აღენიშნება ძილიანობა, დაბნეულობა ან ძლიერი სისუსტე;
